Where filler belongs
The face loses volume unevenly. Fat compartments deflate at different rates, ligaments loosen and the bone underneath remodels, so a hollow temple and a deepening fold are usually two symptoms of the same change rather than two separate problems.
That is why we treat the area that lost support rather than the line you can see. Filling a nasolabial fold directly is one of the most common ways to make a face look heavier instead of younger.

Lips
Lips are the area where restraint matters most, because the difference between a good result and an obvious one is a fraction of a millilitre. We usually treat shape, border definition and the way the lip sits at rest before adding any volume at all.
A first lip treatment is deliberately conservative. Adding more two weeks later is simple; taking it back is a second appointment and a dissolving agent.
When we would choose something else
If the concern is descent rather than deflation, filler will make it heavier. Tightening or a surgical opinion is the honest route there.
If a whole region has thinned rather than a defined area, a biostimulator often reads more naturally, because the change arrives gradually and looks like your own tissue.
We would not treat over active infection in the area, or during pregnancy.
Common questions
Is filler reversible?
Hyaluronic acid filler can be dissolved with hyaluronidase, an enzyme injected into the treated area. It works within a day or two. This is the main reason we use HA products rather than permanent fillers.
How long does dermal filler last?
Between six and eighteen months depending on the area and the product. Areas that move constantly, such as lips, turn over faster than structural areas such as the chin or cheek.
Will lip filler look obvious?
Not if the amount is right for your anatomy. We treat shape and border before volume, and a first treatment is deliberately conservative so that more can be added at a review rather than dissolved.
